CSLA requires that new patients complete and return the "New Patient Intake Forms and Policies" prior to
scheduling an initial evaluation. This is so that our therapists have enough time to review and prepare for your child's first visit. Completed forms and all supporting documents can be emailed to front.desk@chesapeakespeechlanguage.com

It is recommended that forms be completed on a computer to fill out and sign all required fields. Thank you!
